I am a 40 year hospital healthcare practitioner.
A clinical trial, without some confounders and bias, is a rarity if not a unicorn. It can indicate where possibilities may lay, but as a singularity, should NEVER be taken to infer causality. Outside validation by more and similar trials is how we begin to elucidate truths.
The medical system and clinical trials focus and funding have now largely been commandeered by profit and greed. In the late 90s, direct to consumer marketing of medications began with TV advertising. Trials that should have been undertaken were never to be, as no profit, no funding.
Over the years more and more, I felt as if I was given a cattle prod to move people through, in sick care and death care, but not true prevention and wellcare.
I have thus stepped out in early pseudo-retirement.
The critical thinking mindset of practicing professionals has been covertly and actively quashed.
We aren’t free to practice because of liability, and our institutions demand that all our actions must be soundly “evidence-based.” Dissenters are quietly punished behind the curtains.
Those that do not comply are shoved out.
