The appeal of these games is the replayability that comes from that freedom. Fallout 1 & 2 do it well. Arcanum captures the differences between playing as a man or woman the best out of all I know. Even Torment, which has no character appearance (or name!) customisation allows you to play in very different ways that affect how the story develops.

In short, it works if done well.
