Had quite a few clients that had problems like that, either they were too busy or spoiled their dogs usually though. Positive reinforcement's the best method imo, it reduces miscommunication and is a mutually beneficial relationship, but you've gotta be able to find a treat (food/attention generally) that works and keep it exclusive enough to be worked towards.

He gets treats. But again glass stomach from inbreeding it just causes diarrhea

Sounds like a special needs pup to me, there are plenty of german shep rescues though. Luckily it's a very popular breed. Maybe get nt touch with some and offer to foster him while they look? He won't have to even touch a shelter that way.
