The average shareholder of a S&P500 index fund has no clue how rampant incompetency is inside an average company in that group.
The 80/20 rule is alive and well in giant companies. 20% of the people do 80% of the work. The other 80% are either close to retirement, complete idiots, or are part of some small or large amount of fraud or time theft.
