There are several different approaches, including NIP 17 DMs which exist in several clients, relay-based groups (see chachi.chat and groups.nip29.com), relays-as-groups (see flotilla.social), and nostr-over-MLS for nuclear grade privacy (see whitenoise: https://github.com/parres-hq/whitenoise). I'm working on flotilla and would be happy to help you set something up if you're looking to do that.

Yes, its a difficult problem, specifically the encryption, but there are numerous proposed solutions in progress.
I'm working on voistr (nostr softphone) https://voistr.codeberg.page which enables direct peer-to-peer WebRTC connections between nostr users. Right now it's only for audio calls but there'll be instant messaging features too soon. Once the connection is established, peers send encrypted nostr events to each other without needing any relays.
I use Cheogram. XMPP is it's own protocol. I pay $5 a month for a phone number using bitcoin.
I use nostr:npub1exv22uulqnmlluszc4yk92jhs2e5ajcs6mu3t00a6avzjcalj9csm7d828 for messenging. Works fine for me but does require both parties to have simplex on their device.
Can send messages, photos, videos and is anonymous.
