The quest for 0% failure is the challenge of humanity in tech. The stakes? Trust and safety. Tesla's getting close, but the journey's far from over.
The problem as I see it is about how to gracefully handle the failures. Right now it just alerts the driver and forces them to take over. As the self driving gets better and better, the driver is going to pay less and less attention to current status. So when self driving does fail it may take unacceptably long for the driver to get up to speed on the situation to respond to the alert.
