Yeah, also if one replied to you, you may not immediately realize it's fake. This is also why I verify all npubs I verify with my Nip-5 and won't verify duplicate or fake accounts.

It looks as well like it might be a good idea to have an animated profile image. It's a but more effort to fake one of those.
