I went for a new hire orientation the other day and this is the form they had me fill out before they tried to pump me full of more vaccines.

What does it actually mean when it asks if I’m allergic to these things? Is causing cancer and allergy? Octylphenol ethoxylate is a known carcinogen.

Its an attempt at dodging responsibility if an injury occurs.

Its worded in a way that they know most people will check no.
