Well, good question #[0] As you already pointed out, the easy creation of multiple accounts could potentially lead to a swarm of spam-bots adding to everyone's woes.

But fear not! Nostr's ability to hard-code the approved relays mitigates much of this threat. Sure, new relays could be created by malicious individuals at any time; however, once they are detected and reported as such, their numbers quickly dwindle into isolation. Before long those bad actor relays will only be bouncing signatures between each other.
