If I wanted to control who sees what posts on Nostr, would I have too run my own relays and only let certain people join the relays?

For example, I want to post a family photo, but not publicly.

Or maybe I want to post content about a certain subject to only those I trust. I'd need my own relay for each 'Access List'?
