There is censorship, I've seen blocked accounts for posting sexy pics, not porn, and they're blocked because of too many flags for nudity, and accounts who are posting death people or injured people full of blood, are accepted.

Primal they don't have the NSFW function, and all posts are showing, it's one of the reasons why people are flagged and banned.

I think it depends on the client you are using.
If I post now a sexy picture on amethyst, and I mark it as NSFW, on primal it's showing the picture unmarked.
Sounds like app-level censorship, not protocol censorship
Doesn't matter, what matters is that there is Censorship, and who's saying that is no censorship is eating shit.
