The approach I had in mind was that paying users are paying to see what their follows have to say, so they are in a sense sponsoring their follows. This is not perfect for these negative cases - after all you might be following the suspicious accounts to track what they are doing but if you are their first follow ... that is suspicious.

If Alice gives legitimacy to scammer Bob, maybe Alice is a scammer, too? Or she's just gullible. Punishing her a bit and more on repeat violation makes sense in nostr, too. This is an approach a relay that I would want to use (or actually provide) would apply and doesn't mean censorship as Alice can run her own relay or whatever but my relay can't be forced to tolerate promoting scammers.
