The spammer is only using 3 relays. At least they were last night.
And just because you don't have spam doesn't mean it's automatically a you or me problem.
The spammer is probably spamming the top 50 accounts or so. That's what I would do. Spam people that will complain and talk about it to get the most eyeballs on it. That way something gets done about it.
Relays should do a better job, but clients could also require sats before displaying any DM to the user.
