Alzheimer's and dementia are microtubule dysfunction rather than the pharma inspired boogeyman of amyloid plaques. That's why their treatments don't work.
The only approach I have seen results from is low intensity pulsed ultrasound (LIPUS) coupled with L-Threonine.

? Wouldn't the pulsed ultrasound work like lithotripsy for kidney stones?
I believe it is somewhat similar, yes. But the idea behind LIPUS is stimulating microtubules rather than breaking down amyloids.
Ultrasound is also increasingly being used and studied for ALS and Parkinson's, and it's suitable for vagus nerve stimulation (VNS) among other organs.

That's unfortunate. I have personal experience with using it for dementia with growing success for my mother, red light therapy + ultrasound in combination. One family member who is suffering from Parkinson's has been reluctant to try it despite no downsides or risks, so I can't even anecdotally corroborate the claim.
