Using a self custody LN wallet and have opened a new payment channel in the past and sent several transactions through.

Sent sats from WoS.

It created a new payment channel and was successful.

What would cause it to create a new one vs using the other already created one?

because the channel doesnt have enough liquidity

there needs to be space to receive the sats and if you haven't sent out sats yet from the first channel/opening-transaction than it's to full

Ahhh okay. So I need to keep X amount on each side then? Or is the channel not just my two “wallets”?

Yes if you received several inbound payments and didn’t send any back out then the channel balance would all shift to you. Once that happens you can either send some back out to create more inbound capacity again, or the wallet will create a new channel for you. Which wallet was it?
