Let’s say the whole world goes nostr. There are billions of active npubs. I want to find the outbox/inbox relays of someone. How does nostr go about this?

I cannot query all the possible 10002-kind relays (partly because it’s inefficient but mainly because I just don’t know them all).

DHT over pubkeys ? Are there other solutions?

it would be interesting to add nostr clients a kind of "cache relay" for some limited datas like it works with torrent network and distribued files. This could make less charge for the relay, but it must be secured by signature to be sure the cached content is not altered by the nostr client. I'm sure some data can be distribued without involving loudly each relay. Remember #bitcoin make miner interested to mine with the value of bitcoin. Relay on nostr haven't all this reward back for their effort. There is also a concern about time to keep the cached content, but it will depend on the category of the content. Friend list is much more updated than "old" notes.
