I think the problem is that the basic rule of respecting the teleological purpose of sex is muddied by people's desires, well- and ill-formed. People aren't thinking "is this inline with the way we are made and meant to act?" They think "I want this," and more complex but clearer rules need formulating.

Honestly, as long as it elevates the existence of both the man and the woman, it should pose no problem. Both man and woman should enjoy it, and should want to enjoy it. Otherwise something is wrong.
But at the same time, neither should degenerate into whoredom.
E.g. sodomy was prohibited for a reason, even among married couples. It is degrading for both.
