i would like to quit my phd but there is a startling mismatch between my qualification, my experience and market demand

i am a relatively senior doctor. 2 years houseman (resident) + 5 years medical officer. but because i was engrossed in public health pathway, i lack antenatal skill & aesthethic skill. unfortunately, private general practitioner wants this skill.
they are willing to take me but the wage will be small (not a senior doctor kinda pay).
unfortunately my public health skill & certification is in a mismatch with the private industry needs. fortunately i have my master which i can convert to OHD. but if i do my phd (drph), i will be further deviate from market needs. market prefers papers like CMIA, MRO, CHRA, NRA and many more
i can do all these papers after my PHD. but then i will be a junior specialist in that field all over again. wasted 5 years. plus big debt for my phd
problem, market outlook for private doctors in Malaysia is grim. most clinic want to you work 6 days a week with pay which i find miniscule
