Yep, but imagine a special relay that anyone can post (with a paid user tag) in it, but only that paid user can download his citations from it with a NIP-42 AUTH.
For DMs, for instance, if you write to me, I will point you to my special relay that will receive your DM and only I can download it.
If the new GiftCard DMs idea picks up momentum, we will have millions of encrypted packages with user tags. Those messages don't need to be public. They are useless to everyone else. They can all simply be stored in each user's preferred private relays. There is no need to widely broadcast them.
And if that preferred relay has paid read access only, privacy gets even better.
