I'd say that porn addicts fall into two camps. Either they are content with wanking to harder and harder core porn, or they are the sort that isn't happy just wanking; they need a woman to enact their fantasies upon. As you say, sex with a woman (or a child in the case of paedos) is not the same as masturbation and they know it all too well.

This is how I view the incel phenomenon. They could wank to porn all day, but they *need* a woman to have sex with. They hate that they don't have a gf to have sex with.
