I think is forces a full repull/refresh to all configured relays.

Ok makes sense. It not seem to bring back any old follows here, but does seem to refresh some content in my feed

It seems that metadata is synced to relays, so perhaps that could help. Not close enough to the implementation to know.

It is but I've broadcast my metadata to many relays so its really confusing why it nuked my follow list

I had the same issue. Went from 800 to 300 multiple times.
