So what prevented fire ladders and crown fires between the megafauna extinction and the last couple hundred years? I know some Native Americans managed forests but it seems hard to believe there were enough of them to manage all of the North American forests.

Nothing. The answer is nothing. We now have (and have had) forest fires that are so destructive that they kill everything in their path. Only the fire crews have a shot at containing this now.

Native Americans set fires to control vast acreages but, you're right, there were not enough of them to control everything.
