Confession: I don't understand how the outbox model works.

If I self host a relay and I'm the only user and I set that as my outbox how do other clients know to get my notes from there?

If I set a paid relay as my inbox does it accept replies to my notes from users who don't pay for the relay?

Somebody has a good answer on this question? Would also help me to understand nostr better.
