looks like you have an erysipelas. clean the entrance wound. consider i.v. antiobiotics if you are in a bad state. otherwise p.o. could work. depending on your location/allergied: penicillin or cephalosporin. streptococcus group a would be my first guess. also check for general causes: e.g. diabetes meillitua.
