I'm using Bitwarden as a password manager. Works well, has been recommended by a lot of #spirko guests.

I have VaultWarden (Start9's version) on my server and I've wanted to switch to it. But I have been having major issues connecting to it over .onion. So I haven't moved to it yet.

I use KeePass on my phone and computer. Often I'll change a password (or add an account) on one device and I manually copy-and-paste the datafile between them every now and then.
