This is almost always an issue with key packages of the person you’re trying to add. The other party needs to have a key package list event (kind 10051) published and a recent key package.

You can go to developer settings in the menu to delete old key packages and publish a new one if needed.

It’s definitely one of the most brittle parts of the system right now but we’re working on making it better right now.
