My first impression is that it was weather related. Pilots who are not trained to fly in instrument conditions can easily and quickly become disoriented and enter an unusual attitude. The breaking up of the helicopter before impacting the river could point to this. Unusual attitudes can place high stress on the airframe beyond certified limits. Sad for the occupants.
