Do we have an NIP for encrypted images/videos in a DM? Only the two people in the chat should be able to decrypt the image.

Do image hosts even take encrypted files?

Are DMs actually direct?

Perhaps the relay-layer connection should just be to establish a shared encrypted "direct" (tcpip) connection

No, msgs go through relays, but encrypted.
