General practice:, Once in a while I check connected relays to see if they are active, how much spam there was reported etc. If there's a bigger problem I simply remove a specific relay (sometimes adding it back after a while, sometimes not). Generally trusted paid relays are free of spam. They don't cost much. Consider the one from nostr:nprofile1qqsfff483fdwhwayp0g64g3rfqgpxtpdsqzthythv9kyz0fuph0nyrspzamhxue69uhkymmnw3ezuct60fsk6mewdejhgtcpzamhxue69uhkummnw3ezuct60fsk6mewdejhgtcpzamhxue69uhkjmnzdauzuct60fsk6mewdejhgtcw0q5e5 or / and nostr:nprofile1qqst72lw22q3f8rux5846y4wxt63f3uxflcsspgc9aqhs5uv93ppqpcpz4mhxue69uhk5etvd3ukv6tndqhxcctwvshs0lw8yj (a new project, personally haven't tried them yet, but it it looks decent).
