Visual Studio with Cline so far tends to terribly fail with every task that involves dozens of files.
Cline is quite a pain ☹️
And with my project, the api calls add up quickly ... and make Cline freez again or is it normal it doesn't react for 5 minutes straight? Might that be ppq losing messages along the way or is it the model not answering or is Cline actually crashing?

Did you try continue dev? It's free open-source and you can run it locally with self hosted models through ollama.
I'll give it a try but given my project has more than 20k files I have little hope it would not run into unnecessarily gigantic queries that cost a lot.
Sonnet with Cline is indeed expensive. We are always looking into if we are doing something sub-optimal which is costing user money. One possibility is that caching isn't properly happening because our system jumps around with providers. We will look into this.
