I just pick off the suckers as they appear. Then I trim the ends of the vines when they reach the top of the trellis.
Do you cut off the leaves below the flowers/fruit?

No. I've not heard of that. Is this supposed to help?
My observation is that what most people call "diseases" in their tomatoes are just old leaves. Cut all the old leaves off below your fruit and flowers.
The same thing goes for Cucumbers and Zucchini vines
