When I traded with Interactive Brokers there were always piles of bids and asks at different price points. If there is not enough bitcoin to supply to potential buyers, the price of BTC goes up until there is. Market makers don't need to hold any securities, they can get their several pip spread by putting together buyer and seller that overlap and taking the difference for themselves.
