Paid relays are very good. I don't see the spam you are referring to for a weeks already.

I only use one paid relay: wss://nostr.wine
As for the WoT relay, it depends who you trust. You can pick some from this list: https://github.com/bitvora/wot-relay?tab=readme-ov-file#available-relays
I hate to say this. but all measures we have now are barely effective. I have been trying to figure this out, but its very difficult.
Best way to take all of this out. move to paid relays.

Right click on PFP of spam and copy the name of the file
Go to settings, security, hidden words
Add the name.jpeg
