I got the nsfw one active, but surely there must be more one can do to avoid certain content?

Depends on what you find nasty. I’ve found some people can speak words that are cruel or offensive & it’s been best to mute them.
1) Mute individual npubs, hashtags, and words.
2) Adjust relays.
