How do we increase privacy in E2EE Nostr DMs?

We decentralize further. We can privately tell each DM peer to use a different relay and key for each message in the chat. In that way, your trusted relays only capture part of the conversation and, if setup correctly, they can't correlate all of your keys as one identity.
