What I'm worried about is child porn on nostr. How is that going to be stopped.. there could be relay specialized on that kind of nasty stuff in the future.

Doesn't need to be stopped at the protocol level, you can send child porn over email right now.
Law Enforcement just needs to do its job, which is finding and arresting actual pedos instead of policing the internet.
