You can always keep your feed clean, just follow the people you want and use your own relay. However, if you're using a free relay, you end up becoming the product. Even the trending system is interesting: you can upload your own content, but if it doesn't receive many interactions, it will eventually drop off, even if you have satoshis to pay. The content needs engagement, such as replies, likes, etc., to stay on the list.
