That's strange. Are you using any other client? There are some clients that don't understand the hashtag follows and endup erasing them from your follow list. Other than that, I haven't seen a bug on this for a while.

Ohh I just saw that recent follows/unfollow are not updating the top bar list, but if you kill the app in the Settings/Apps, they will show up

I have played around with primal.
I just opened primal then came back to amethyst. All hash tags were gone
