Based on the explanation I heard, Kingdom Hearts is the collective "heart" of existence, it's kind of like The Warp from Warhammer 40k. An example is at the end of the first game where Ansem is certain it's dark but then Sora says it's light and Ansem gets blasted; they were actually both right, it changed because of the strength of Sora's belief.
