You had to of seen it to know to block the user. Meaning you need to wipe your browser or app to remove it from cache

Nostr clients fetch everything from relays and then processes your settings.

We dont know for sure the state of the images. Need a dev to chime in

I blocked it with AdGuard so it never downloads to my network
