You don't have to pay in Bitcoin to pay with Bitcoin. It's called foreign exchange and it's part of what makes a currency a currency.

Do you convert as needed or is there a service where you are that converts as you spend?

I have Bitcoin debit cards I can fund as needed with a couple of clicks.
