The case involved proxying requests to a Heroku app through Nginx, resulting in a "Not Found" page when accessing the custom domain. By reviewing the Nginx configuration and testing different scenarios, the issue was ultimately traced back to the Host header configuration. The solution lay in setting the Host header to the correct value, namely the Heroku app's subdomain.
