I think the next real Nostr challenge will be for clients to get smarter about data management.

Still think the buffer relay might be a good solution, some trade offs with hub and spoke style “centralization” but users could still pick different buffer relays to choose from.

These relays would aggregate and many-many relays for clients to pull from a single source and avoid data duplication.

This might also help with the avatar IP leaking.
