A reaction is just another kind of event, with it's content being an emoji.
As per NIP-25, it's not restricted to kind1 notes & can be used to react to any event kind.
(You can even react to reactions, probably if you use a CLI client 😅)

I never implemented this because I felt like it leaked private data a bit more than I would like. The metadata is bad enough.
