I was told about Roatán and Prospera. All above should be included. It is far, though.

I got the impression that it used to be like that, but they got more down to earth lately... I never been there so I can't really tell you more than what vibe I got after seeing a presentation on it last Sunday. Surprisingly Rwanda should be nice as well, I have a friend who lived there for a couple of years. But again I never been on any of those places. From places I visited Albania is probably the closest to what you are looking for, but it still does not check all your requirements.
