Sometimes profile settings don’t get published to ALL your relays. And luck of the draw Damus gets the profile image from the relay where it is missing. I have this frequently when updating my Lightning address and still get zaps to a wallet address no longer (apparently) in my profile. Resolution is to reset your profile image again, save it, and hope all the relays are behaving.
