What clients are you using? Most likely one of your clients is borked and causing issues with the rest.

I think I've heard of people using hist.nostr.land or something like that to deal with this
Try to narrow down which nostr client is causing it and stop using that client. To backup your follow list you can occasionally visit https://metadata.nostr.com or subscribe to Primal premium.

Follower recovery can be done using https://hist.nostr.land
As for how to prevent it happening in the first place, the main thing to do is only follow/unfollow people from ONE client, and don't completely change all your relays. When using a different client, before you follow someone, make sure your relays match the client where you are usually following/unfollowing people.
I had the same thing, all following was gone. Using Damus. I did one new thing that day and that was playing around with nostrudel on my umbrel.
