i thought about and even filed an issue towards the end of adding encryption to NIP-42 so it can transport through relays that act as proxies

but then i realised, wait... this can be something else... so if i have a paid relay, there can be a special type of event called "proxy" message, a kind, and inside that event is an event that creates a NAT style masquerade forwarding over a websocket so the paid relay acts like a proxy delivering this and returning responses

gotta think about it a bit more but i think that layers always > complex interfaces, so i am sure this is the right direction to go
