How do you only have 16 when I have 148 muted and still super noisy 
That also probably depends on the client you're using and which relays you're connected to. From what I understand, Iris recently started limiting what you see to 4 degrees of separation from your account. So people you follow, people that they follow/follow them and so on to 4 degrees.
