It's complicated to implement the solution for this thing still due to numerous of complications, risks, and tradeoffs at the same time.

At certain part, Technically, the network is decentralized. But the relay is still centralized as you suggested. That's correct and i agree about it.

For the filtering or censorship part, It's mostly still happening till today not because of persons that relay owner's hate. These are quite different from fediverse. But we are mostly having to deal with legal troubles such as legit illegal content that is actually prohibited and strict by law. So it's complicated to keep these at the same time.
